Not sure if anyone knows the solution, but I can't find anything on Google that remedies this issue: After installing Office 2016 for Family on my Mac, every time I launch an application, the Keychain dialog pops up with 3 choices: Always Allow Deny Allow However, NONE of these options do anything. I click on each one and the dialog just stays there. The only way to get rid of it is to right click on the running app (usually Powerpoint or Excel) and then choose Force Quit. I even tried removing Office as suggested in another forum and then reinstalling it. Still the same problem. I'm running El Capitan on a MacBook Pro. I have the same problem.
I have to 'deny' for all MS apps between 3 and 5 times before it goes away. I spent 4 hours on phone with MS and 1.5 with Apple. I am hoping that they fix it in the next updates on both the OS and Office. It started for me after I tried to use a SharePoint team site belonging to a client. As soon as I had to change MS Office 365 credentials, my MS office 365 started to freak out. I now have to use the SharePoint site through an Amazon Workspace. These are not obscure issues - they would arise for lots of Mac users.

Why is it taking so long to solve. Rather than Shut Off KeyChain. Instead: • Open Key Chain Access. • In the Find Field at top of Window that opens, Type Office the hit return. • You should see at least three items starting with a code number (a combination alpha-numerical characters, and in description makes Reference to Office 15.2 or newer) • Delete these items (make sure items are only associated with Office version 15.2 or later) Do not touch anything related to office 2011 which would be version 14.5.x (x being a number).
• Now Quit Key Chain Access. • Now open Word, Excel, PowerPoint and One Note.
• Sign in and if it's necessary to enter access Code you were sent over your cell phone enter it. If you did everything required correctly you should now be signed in and should no longer see your message about never, allow, allow always except the very first time you sign in. In that case choose always allow only for this item. • Repeat as necessary for each application. Once setup then you should no longer have this issue.
Note: Recently there has been a new issue where after a New upgrade you have to go into Key Chain Access in these same files click on the get info and Then unlock and change password. For some reason it gets corrupted or overwritten They are not supposed to mess with the password for any reason.

Microsoft told me to delete this key and restart my Mac. Make sure your apps are closed. Two new keys were created, Microsoft Identity 2 and Microsoft Cache 2.
I reopened my Excel and then a recent document and the keychain popup was gone.
